Patient Information

All patients are asked to complete a medical history so that you may be given treatment best tailored to your individual needs. If you are taking any medication please ensure that accurate and full details of these are added to your medical form as some may affect the choice of treatment plan provided.

 If you have any special requirements, wishes or desires for your teeth please don’t hesitate to mention this because it is amazing what can be achieved.

You will receive a full examination of your teeth and surrounding soft tissues and appropriate X-rays will be taken. A treatment plan will be formulated and discussed at a subsequent appointment.

You will also be given an estimate of the cost of your treatment. It would be very helpful if patients state when making appointments if they have a problem coping with stairs so they can be accommodated on a ground floor surgery, where it is possible to take wheelchairs right up to the chairside.
